Waiver of Signed Consent for Child Participants

Thank you for your interest in our study entitled SEALS (Study on Emerging Abilities in Language and Speech). For this project, we are recruiting children from birth to 3 years of age who have motor delay or a medical diagnosis related to cerebral palsy. The information we gather from your child will help us to identify and characterize clinical features related to the onset of first word production and predict subsequent growth in speech production in children with motor delay or CP.

If you are interested in potentially having your child participate in this study, we ask that you provide us with some basic information about you and your child so that we can determine whether your child is eligible for the study and so that we can contact you later on to schedule an appointment.
